1. Review the following qualifications to determine your eligibility:

• Do you have a disability?

• Do you qualify for Social Security, Medicaid, or food stamps?

 If you answered yes to both of these questions, you might be eligible.

 

2. Give us a call or send an email with the following information:

• Your name

• How we can reach you (your email, phone number)

• Name of your Region Ten case manager (if you have one)

     

3. We will be in contact with you for next steps on being added to the waitlist!
